A distinction should be made between exhibiting occaisional narcissitic behavior - which everyone does to some degree - and having Narcissitic Personality Disorder as defined by DSM. Without getting into a very lengthy point-by-point discussion, Angelus behaves in ways that fit more with the colloquial definitions of narcissist than with the medical definitions.

If we were going to look at the characters and assign DSM personality disorders to them based upon behaviors and relationships - IMHO Angel would be more like the case of Depression with some hints of Borderline Personality Disorder whereas Spike is a better example of a patient with Narcissitic Personality Disorder.
