I am pretty certain that all that is ever mentioned is the part:

Dru: " I miss Prague"

Spike: "You nearly got killed in Prague!"

I highly doubt that Spike left Dru in Europe and went to the US on his own in the 70s, so I guess there are only two options:
The Prague thing happened before the 70s and her sickness lasted pretty long, up till the late 90s.
or they both went back after NY and it happened right before their appearance in Sunnydale.

Also: I don't think the flying is such a problem. With no need of Oxygen and all they can always hide in the cargo part of the plane....even Kendra managed to do that.
