Whether Buffy knows what Cecily said is not important, but I think it is clear that Spike let her know why Cecily refused him. The writer doesn't do such things, by accident. It's to remind us of what came before. It doesn't mean that Buffy is as callous as Cecily, but it does mean that at that point in the story she wants him to stay away from her. It's not that odd that a person in stressful situation would say the most cruel things they can think of, and at this point of the story Buffy was normally saying cruel things to Spike.
