Don't beat yourself up over the boy's problem; you can't know everything. Most issues do pass on their own in time.

You might want to consider allergy. Has he recently started using any new products—soaps, shampoos, etc? Are you using any new laundry or cleaning products, fabric softeners, disinfectant sprays, etc? Two kids in my extended family have had some really crazy reactions to chemicals found in all sorts of household products, and even some foods. One young girl has hideous reactions to sodium laureth (or laurel) sulfate. She can't get near the stuff! She collapsed in the shower the first time she had a reaction. Apparently she'd used it many times before finally developing a reaction.

Another, a boy, can't use anything that has benzoate or benzyl, or anything with the prefix "ben..." in it. It's apparently a preservative used in many products, but he had all sorts of ugly reactions from it for years until finally learning what was causing them. Good luck.
