Modern Trends in Well-child and Well-teen Assessments - as They Manifest in Certain Areas in th World

There is a growing tendency in many societies to call in children at specific age levels for a thorrough assessment of the health and development with advanced radiological and endoscopic methods, much of which is perfomed through the openings in the intimate area. Usually the child is put under anesthesia for this exam. It happens in USA, Canada, Latin America, Western Europe, Australia and all other coumtries with strong cultural and political ties to USA and UK. It also happens in Eastern Europe but with somewhat other methods.

This ordeal is typically performed in a health center or in a hospital, and takes typically 3-5 hours, and is usually done some days after a more traditional well-child exam, a psychological assessment and an assesment by a dentist.

Some avatguardistic societies even hospitalize the child for a couple of days or have a hospital department within schools where the children stay for a couple of days for this type of examination.

The elected age levels for the ordeal tend to be 6-7, 11-12 and 15-16. These ordeals follow an internationlly standardized protocol and are coordinated internationally by some orgaqnization, nost probably the World Health organization (WHO). But this intrnational exam and surveillance program is mostly held secret for the common public as much as posible.

The following is a description of this protocoll.


PREPARATIONS AT HOME BEFORE THE EXAM

The parents will often be instructed not to give the child food after some time the day before and to give the child some laxative agent to make his stomach empty. They are instructed to bath the child and bring himher to the exam only lightly clothed. In some communities the child is taken to the exam from home or school in a special ambulance.

Some communities use huge ambulance vans that drive around, pick up the kid and perform the whole ordeal inside the van, that usually will be parked in a shielded area until the exam is finihed.


EXAM INITIATION

The exam will typically start as an ordinary well-child physical with the purpose of making the kid believe this willl not be something especially intrusive, and gradually increase the intimateness and intrusiveness of the ordeal.

One will perform a structured conversation with both the parent and the child. During this part the child will still have most of the body covered by clothes, but lightly so that bodily reaction during the conversation can be observed. This part may however occur in the home of the kid or the school before the kid is taken along.

At the end of this preliminary part the child will be clothed down to underwear or given a gown or some light exam clothes that are easy to open or remove.

At this early stage, the kid will usually be given some anesthesia through the mouth, by a shot or into the anus to make himnher calm and easy to manipulate.

Then one will listen to the heart and lungs, take the temperature, pulse rate and blood presure.

Then all clothes will be taken off the child, but heshe will typically be offered a gown or carpet to cover the body parts that are not under examination.

The exam is likely to proceed with anthropomorphic measurements with an assessment of the growth patern. This stage will include measurement of weight, height, limb length, and circumferance of all body parts.

During this early part one will also inspect the mouth, nose, genitals and anal region and take specimens of micobes and secretions from all body openings.


ORTHOPEDIC EXAM

The next stage will be a thorrough orthopedic examination, where the shape, flexibility, function and development status of all muscles, joints and bones in the body will be assessed, including the whole pelvic area and the muscles in the pelvic floor. The child will stand, sit or lie according to what is most practical for the different tests.

This part will typically start like the orthopedic exam in an ordinary well-child physical.

This examination will include:

-A check of spine curvature to diagnose scoliosis or other deformities, performed with the child in standing and foreward bent posisions.

-Checking of flexibility in the schoulders, armes, spine and hip with the child in standing and sitting posision.

-Assessment of leg and hip joint flexibility with the child laying, performed by probing all possible modes of bending and twisting of the legs.

- A general inspection of the pelvic structure with the legs bent up and spread, including assessment of the alignment of the rectal opening, symmetry of the buttocks, and symmetry of the outer genital structures.

-Ultrasound and X-ray examination of the major joints in the body, especially the hip joints, knees, shoulder joints and jaws.

During all of these eacminations, pictures of the kid in various positions will be taken and stored in a database.


ABDOMINAL AND CHEST EXAM WITH EXTERNAL MEASURES

After the orthopedic examination there one will perform an outer abdominal assessment whith the following procedure.

- Manual palpation and percussion all over the abdominal and renal region with the child laying on the back and laying at the sides with the legs flexed, to assess the shape of the abdominal organs, detect any tissue changes, painful points, muscular tension, masses, fluid accumulation and abnormal accumulation of intestinal content.

- Ultrasound examination of the abdomen with special attention at the bladder and the renal area.

- Ultrasound examination of the chest with special attention at the functions of the heart and great blood vessels.


FURTHER PREPARATIONS AND ANESTHESIA

As preparation for the following endoscopic exams, the child will usually get an enema and the intimate area will be washed. The enema will often be performed with a machine that flushes the colon several times through a tube inserted through the anus.

They will then place the child on the examination table. They will place monitoring electrodes on the head, chest, pelvic area and thighs. They will insert an intravenous tube into a blood vessel.

Then they will give the child anesthesia. The anesthesia will most often be initiated with medication given through the IV tube and in many societies this will be the main anesthesia throughout the procedures to follow.

But some societies will proceed by giving general anesthesia with artificial breathing. In this case the child will be given gas with a mask until heshe is uncoscious, then the breathing will be paralyzed whith medication through the IV tube, then a tube will be inserted into the airpipe of the child and through this tube the child will resceive further anesthetic gas and artificial breathing.

When the anestesia is effective, they will position the child for the endoscopic examinations. Hisher legs will be flexed, spread and placed upon stirrups to fully expose the pelvic area, and the arms extended to the sides or flexed upards at both sides of the head. The child will be held in this position for most of the endoscopic procedures.


EXAMINATION OF THE GENITALS

Then there will be a more specific genital assessment with the child laying on the back with the legs flexed up and separeted. The exact posision will vary according to the development stage of the child.

For girls this will include:

- Manual palpation of the outer genital area

- Examination of the genital area with an outer ultrasound sond.

- Separation of the labia to inspect the inner area of the genitals.

- Inspection of the vagina with an optical scope or using a speculum.

- Examination of the inner pelvic structures with an ultrasound probe inserted throughh the vagina or anus.

For boys the genital exam will include:

- Manual palpation of the penis, scrotum, testicles and the area down to the anus.

- Ultrasound exam of the scrotum, testicles and the area down to the anus.

- Retraction of foreskin and spreading uretral opening for inspection.

- Taking specimen of prostate fluid with a tube inserted into the urethra, and if necessary with a massage of the prostate with a finger or instrument inserted through the anus.

- Palpation of the prostate with a finger inserted through the anus.

- Ultrasound exam of the prostate region with a sond inserted through the anus.


INTERNAL INSPECTION OF THE URINARY TRACT

The child will then undergo cystoscopy where one inspects visually the inside of the urethra and the bladder, and possibly also takes small specimens from the inside walls. For boys there will be a thorrough inspection of the prostate area during this examination. Traditionally cystoscopy has been a difficult examination to undertake, but modern flexible instruments ha made it to an easy and very quick examination. During this exam the bladder will be filled with fluid and in that shape also assessed with an outer ultrasound sond.

Often one extends the inspection bu advancing as cope up into each kideney.


EXAM OF THE RECTUM AND COLON

Then they will examin the anus, rectum and colon, which involves.

- Examination of the anus and interior room with an inserted finger

- Opening of the anus with a speculum for visual inspection and taking specimens.

- Internal ultrasound exam to visualize the structures around the rectum.

- Inspection of the deeper parts of rectum and terminal colon with a flexible optical scope.

- Often one extends the inspection to include the whole of the colon and even the terminal part of the small intesine.


EXAMINATION INSIDE THE STOMACH THROUGH A HOLE MADE IN THE NAVEL REGION

This exam consists of making a small perforation in the botton of the navel, incerting a thin flexible endosocpe and inspecting the inside of the belly with this.

Sometimes one also takes tissue samples from the inner abdominal area during this exam, or even takes out the appendix.


OTHER ENDOSCOPIC EXAMINATIONS AND TISSUE SAMPELLING

Other endoscapic examinations than those mentioned occur during the ordeals, but vary according to a present whish, for example inspections in the nose or down into the upper digestive tract through the esophagus.

During the endoscopic examinations they will sample tissue speciments for analysis. Often they also do so from other regions, for example the area under or around the breast nipples.


IMPLANTMENT OF ELECTRONIC SURVEILLANCE CHIPS INTO THE KID

During the exam, they implant one or two electronic chips into the kid so that the wheareabouts and physiological status of the kid cam be followed around the clock from a secret surveillance station. They typically implant one chip in the chest and one in the pelvic area.

The chips will communicate with the station by use of the network of communication antennas set up thightly in most communities, just like cellphones are followed the same way. This chip implantement is the most secret of all particularities of this international program, but has been revealed accidentally when the kids have been subjected to examinations with independent doctors, that did not know about the progran in detail.



TEST OF PHYSIOLOGICAL REACTIONS

After the active exams, the child will be placed in an intensive care unit, with the recording electrodes in place, with a sond through the urethrato in the bladder, and often a sond through the anus going up into the colon.

The sonds going into the bldder and through the anus are used for drainage, but will will often have ability to record bodily activities and are used to test the functions in the urinary tract and lower digestive system. Typically sections of the sonds will be inflated, and the reactions inside the body will be recorded, or the bladder and the colon will be filled with fluid and stretched to test reactions.

Girls may also have sonds into the vagina, and boys often have devices surrounding the genitals. These are used to stimulate the sexual physiology by mechanical movements and by electric stimulations.

During these tests, the kid is held under anesthesia, but not so that bodily reactions are hindered.

After these tests, the child is allowed to wake up and the electrodes and sonds will be removed after some time.


HOW DATA FROM THE EXAMS ARE ADMINISTERED

The instruments used in most of these examinations will take pictures and measurements that are transfered to a computer, stored there and later taken forth for further inspection and diagnosis. The society will store all of the data in a secret database that authorities in the sodiety dealing with the kids will use accordingh to need.


SOME HISTORICAL ACCOUNTS

The type of exam program described here, is nothing new. It has been attempted from time to time in various societies around the world at least from the end of the second world war. In most of these societies, a certain percentage of children seem to have been chosen for the program, typically 10%. but steadily more societies perform this program on all kids in the society.

About intimate examinations of children in stirrups

In our modern societies over 50% of girls and boys have had gyn-type exams been, often in stirrups before age 7.

But they are usually given general anesthesia or deep sedation, so that they do not remember it very well. But still this gives feelings in the body that make the kid understand something strange have occured to them, often the anesthesia does not work properly so that glimpses can pop up later.

These glimpses can make the child tell strange things to their parents that these do not understand, because they do not know that it is a fairly common thing to place kids in stirrups. Occations where this is done are many, but here are a few.

Many kids undergo cystocopies or urological examinations. For girls these always occur in stirrups, for boys always if rigid instruments are used or if one uses an ultrasound sond in the anus for an overview of the pelvic zones.

Many kids also go through examinations in the lower digestive tract, and stirrups will often be used during these exams.

Many kids go through examinations after suspicion of sexual abuse, and again stirrups are used.

Certain schools, for example alternative schools like Montessori schools, or certain communities, do a development check of the kids at an early age and then again some time before puberty. It is often done under general anesthesia or deep sedation. The pelvic zones are in special focus during these exams and stirrups will often be used.

Kids are often secretly examined in their pelvic zones during tonsil surgeries, dental work or other procedures in their upper bodies when these are done under general anesthesia.

If a kid is placed in a correction facility, mental hospital or the like, for observation or therapy, heshe will almost certainly have a pelvic exam in stirrups, but often under anesthesia, that coprizes the genitals, the urinary system, the lower digestive system and pelvic orthopedics.

In certain areas the health authorities has begun to subject children and teens to extended well-child physicals where one performs several or all of the following procedures: inspections with optical scopes through all or several body openings,ultrasound examinations of most body structures, x-ray taking of joints, EEG and EKG. The kid is often given sedation or even general anesthesia during these

extended physicals to make the kid cooperative and to hinder him or her from remembering. Still the kid will remember a lot of details around the procedures and sometimes also from the procedures themselves, because anesthesia never works perfectly, and the kid will always have some symptoms afterwards that tell a lot of what has been done. SOME EXTREME CASE REPORTS

Even though what is described above, is tyhe most standard way these programs are varried out, one will find that some societies or schools let children be used in a more extreme way during this program. In these cases, the children are used as guinea pigs in more extensive research projects regarding developmentan, new medical techniques and new ways of controling the population, chiche these reoirts illustrate:




Boy, East Germany, full inspection and testing in all body areas from 9yo - 20yo

I am male. It happened in Western Europe. 1970-1979, 1980-1989, 1990-1999, every year from 9 to 20 year old or more. I was only told that i would have a medical exam, no details where disclosed while i was still a child, of course with each of these very thourough exams i suspected i was having another one again.

In a bus in group from our school. At a room in the school that was preliminary prepared for the event. All children in my school of the age group. East germany but i am not aware if it happened in other regions there too.

Preparations before the exams:
I was not allowed to eat some time before the exam. I got laxatives at home to clean out my stomach the day before the exam. They washed my body before the exam. I got a traditional enema before the exam to clean out my stomach. I was totally naked through all the procedures. I had to strip totally naked before the exam. They placed me upon something like a dentist's chair, but with devices to lift my legs onto. I was placed to lay on my back with my legs flexed up, spread and mounted upon two devices. I got IVs, tubes into blood vessels, in an arm. I got IV tube into blood vessels in a leg. I got a device on a finger or a toe to measure pulse and oxygen in my blood. I got a blood pressure monitor wrapped around an arm or a leg.

I got electrodes on my cheast, on my stomach or hip regionamd on my head. I got a tube or wire in through my urethra, a tube or sond in through my anus. For some parts did not get any anesthesia. I got some numbing and lubricating cream injected into my urinary opening and into my anus. For some parts I got anesthesia yhat made me relaxed, but I was conscious.

The relaxing medicine did not take away any of the pain, it made it impossible to move around though. I remember my arms and legs felt very heavy and that i tried moving them but could not do so although i was not restrained on the "dentist-chair.

The doctors never described what they were about to do, was not really necessary since i usually got the relaxant-anesthesia so i was unable to resist anyway - although i never would have resisted. I was raised to obey my elders as were my friends.


Examination with external equipment:
They performed an EEC (electroencephalogram) - functional test of the brain with electrodes on the head. They performed an ECG, functional test of the heart function, with electrodes on my chest and other body areas. They performed an analysis of my breathing and oxygen uptake where I had to breath into a tube. I had to do some physical work, like cycling or running on a threadmill when the ECG and breathing analysis was performed.


They examined my head or neck, my upper body, my stomach area, my shoulder area with, my hip area, my arms and legs with an ultrasound device or something similar.

They x-ray pictures of my cheast, stomach, shoulder area, hip area and of my limbs.

They used electrodes to record functions inside my head. They used electrodes at my chest, at my belly and at my limbs to record body functions.


They examined my genitals with an ultrasound device. They had electrodes at my genitals to record functions in this area.

Internal inspections:
They ispected the inside of my nose with a scope. They took specimens from my nose with a tube. They inspected the inside of my throut with a scope amd took specimens from my throut with a tube.

They inspected the inside of my urethra and bladder with a scope, and took specimens from my urethra and bladder with a tube.

They inspected my anus with a speculum. They took specimens from my anus with a tube. They inspected me through my anus with a rigid scope. They performed a full colonoscopy with a flexible scope. They inspected me through my anus with an ultrasound probe. The doctor explored my anus with a finger.

Surgical procedure performed during the exam, whan I was 13:
I was circumcised in that position when i was 13 years old. I was still not-retractable at the time but the year i was 13 i got a new doctor doing the exam and he was not happy with me and 2 of my classmates being unretractable, so he gave us 3 very low and very very tight cuts and removed the frenulum on each of us. All 3 also had a meatotomy done - optical a very pleasing result, my parents were happy about it.


Arrangements after the exam:
I had breathing tube. I had IVs into blood vessels. I had electrodes on my head. I had electrodes on my body. I had catheter into my bladder. I had a tube going in through my anus. I lay with my legs in stirrups some time after the exam.

I and the other 2 boys from my class were taken back to the doctor about 1 week later to have the sutures removed and the circumcision examined, the doctor also did examine the anus of us 3. Also the catheter we boys had still in our urethas was changed (pulled out and a new one inserted) The catheter i had in my uretha was keept in there for 2 weeks, i believe it was mostly there to keep the meatotomy from growing together again.

Tjes scool I attended:
I attended ordinary daytime school. I went to an evening or weekend gymnastic school.

This seems to be another example from an international project where all body regions of kids are inspected and tested at several age levels. Even though theese projects probably are administered in America and UK, Spoceties in East Germamy higly likely let their kids be used as objects in the projects, mith economical compensation. Such cooperation with Weatern medical industry and authorities is a historic fact.

It seems like this boy got medication that paralyzed the muscles in the body and got artificial ventilation, at least during some of the exam time. Possibly he also got general anesthesia, but that this anesthesia did not manage to put him to sleep, which sometimes happen.

Again they used a dentist-chair-like device to place the kid onto that makes all body openings available for easy use.

It seems like this boy underwent a series of surgical adjustments in his pelvic region, not only circumcission and meatotomy. The muscle relaxant, breathing tube use of stirrups and long time use of catheter point to that. Kids attending gymnastic schools fairly frequently undergo such adjustments to make them more flexible.

Boy, 14yo, North America, body testings and pekvic endoscopies done in School complex

I am male. It happened in North America, 2010-2014, 14 years old. My parents were notified, and spoke with me about it. They understood it would be extensive.I did not understand that.

All children with some kind of health problem had this done.

My parents drove me to the exam, at medical testing facility which is part of the same complex that my school is part of.



There were 4 patients in the room being tested, all of us boys.


PREPARATIONS:

I was not allowed to eat some time before the exam. I got a traditional enema before the exam to clean out my stomach. I was totally naked through all the procedures.

I had to take all my clothes off just before I got anesthesia. I had to strip totally naked before the exam. They placed me upon a simple examination table. I was placed to lay on my back with my legs flexed up, spread and mounted upon two devices, alsp on my stomach with my legs flexed down and spread and I was placed to lay on my right side.

I got IVs, tubes into blood vessels in an arm, a device on a finger or a toe to measure pulse and oxygen in my blood and a blood pressure monitor wrapped around an arm or a leg.I got electrodes on my cheast, on my stomach or hip region, on my legs, on my head. I got a tube or wire in through my urethra. I got a tube or sond in through my anus. Parts of my body got numbed by a shot with a syringe.

I got some numbing and lubricating cream injected into my urinary opening. The anesthesia made me relaxed, but I was conscious. I was mostly awake during the procedure, despite anesthesia given.

I had an IV in my arm just in case I needed something emergency, such as anaesthesia or medication. I did not need it, and the IV was there but not used.

I was fully aware the entire time.


EXAMINATION WITH EXTERNAL EQUIPMENT:

They performed an EEC (electroencephalogram) - functional test of the brain with electrodes on the head. They performed an ECG, functional test of the heart function, with electrodes on my chest and other body areas. They performed an analysis of my breathing and oxygen uptake where I had to breath into a tube. I had to do some physical work, like cycling or running on a threadmill when the ECG and breathing analysis was performed. They did some other type of test with advanced machines.

They examined my upper body with an ultrasound device or something similar. They examined my stomach area with and ultrasound device or something similar. They examined my hip area with an ultrasound device or something similar.

They took chest x-ray pictures, x-ray pictures of my shoulder area. my hip area, my limbs.

They used electrodes to record functions inside my head, at my chest to record body functions, at my belly to record body functions.


They examined my genitals with an ultrasound device. They had electrodes at my genitals to record functions in this area.


INTERNAL INSPECTIONS:

They inspected the inside of my nose with a scope. They took specimens from my nose with a tube. They inspected the inside of my throut with a scope. They took specimens from my throut with a tube.

They inspected the inside of my urethra and bladder with a scope. They took specimens from my urethra with a tube.

They inspected my anus with a speculum. They inspected me through my anus high up with a flexible scope. They inspected me through my anus with an ultrasound probe. The doctor explored my anus with a finger.

They were not painful, but rather strange and uncomfortable.


ARRANGEMENTS AFTER THE EXAM:

I had electrodes on my body. I had catheter into my bladder. I lay with my legs in stirrups some time after the exam. I also lay some time in a bed to recover. I lay naked under a blanket after the exam.


Left some time (perhaps 1 hour) lying on table with feet up, monitors attached, catheter in. Then feet lowered, covered, allowed to rest/sleep. After about 2 hours, monitors and machines were removed and I moved to a bed in the same room, to rest for several hours while tests were analysed.

My mum came to take me home.


THE SCHOOL I ATTENDED:

I went to some school for kids with special needs.

COMMENTS:
This is a omprehensive health check performed according to an international protocol. It is steadily more often used on children as a total screening for problems in most body parts, but with special attention at the pelvic zones. It is also used for research on kids and for surveillance of the kids in the society. It is furthermore sometimes used to harvest body tissue to be used for medical purposes that is not in the interest of the kid itself but for treatment of other patients. Sometimes during the procedure an electronic chip is placed into the kid so that authorities can surveil the kid on a daily basis.

In some communities or some schools around the World the protocol is mandatory for all children. But this battery of examinations are never spoken openly about to the public, not even in communities where it has been made mandatory. In some communities the things done is even shielded for the kid and partly for the parents by use of deep sedation or general anesthesia, but at some places that degree of secrecy is not observed, like this story confirms.
