肩周炎多見於年紀較大者(45-55年齡階層﹐男>女)﹐診斷多為不明疼痛+活動受阻等臨床證據。肩周炎屬於monoarthritis一種﹐如果本身比較少運動/工作中受到外來壓力﹐甚至本身有長期病(如pauci-/poly-arthritis/RFN-arthritis/痛風/高尿酸)﹐頸/肩/背部曾經受創傷等問題都會誘發/加劇肩周炎...

肩周炎好難講會唔會「斷尾」﹐因為arthritis本身大多成因不明同持久延續程度不一。基本上係「痛就要醫」般貓捉老鼠問題。至於「斷尾」後同一關節倘若再出事係唔係一定肩周炎引起﹐定其他問題亦好難講﹐始終無論點醫治方法雷同所以亦好多時候冇分。

醫治方法... comprehensively, we have: medical, surgical, physio, OT

medical: (1)防止發炎﹐有NSAID同類固醇。通常會由輕度NSAID開始(如ibuprofen/naproxen﹐唔好上去高一級去difene...但越上高一級=多一D機會胃出血同腎功能出問題)﹐同樣地﹐類固醇為十分有效消炎藥但亦有其不良副作用﹐所以永遠唔係first line。


physio/OT: 主要係做關節放鬆同防止肌肉韌帶拉傷等物理治療。本身配合藥物能有效防止/減輕痛楚(但不能完全防止其再發生)。OT可以參考你家居同工作上有否有加劇痛症情況而作提議/改善

surgical:用於極嚴重關節炎痛症/關節壞死等情況(所有medical/PT/OT治療方法均無效時)。可能需要total joint replacement(替換換人工關節)
