陳太太﹐你兒子應該係有impetigo...係常見兒科問題。

皮膚上係有好多細菌﹐包括金黃色葡萄球菌(S. Aureus = SA)。SA有部份細菌能夠進化去抵抗抗生素。而我地用其中一種抗生素(meticillin)去測試其抗藥性。如果呈陽性(抗拒meticillin)我地叫呢個SA群體“MRSA”(抗藥性金黃色葡萄球菌)

MRSA唔常見﹐但因為社區中濫用抗生素關係好多金黃色葡萄球菌已呈現抗藥性(由普通SA進化為MRSA)+逐漸多見。因為meticillin一類藥(如amoxicillin, cloxicillin)為廣泛應用抗生素﹐已經唔能夠係MRSA處起作用﹐倘若受嚴重感染後要用更高級抗生素(如vancomycin,
fusidic acid, meropenem等)所以用藥方面亦好受限制。

但請放心﹐SA/MRSA係可以存在係皮膚鼻內咽喉頂等部位而唔引起任何問題。但有時如果菌數太多(如進食時唔洗手)/身體免疫力下降/有傷口等就可能引發局部(如你小兒所有impetigo)甚至體內感染(叫septicemia)了。

一般小兒impetigo係局部感染﹐亦唔會引致體內(因為皮膚系統同血液系統分屬兩樣不同系統)感染。但需要注意清除impetigo...除放膿drainage外﹐我呢邊一般會會開Fucidin cream一日三次一到兩星期涂上患處﹐另加(如需要)chlorhexidine wash(外用洗滌劑)﹐一星期每日一次。不過妳應該聽從醫生指示用藥(我俾只係意見﹐我講兩種外用藥都需要醫生紙買)。

只要用藥適當﹐Impetigo一般係一到兩星期內好番。預防﹕好難﹐細路哥週圍玩你防都防唔到...而且impetigo係好普通兒科問題﹐唔需要擔心。
