A local Muslim told me that, as a diabetic, I would not be expected to fast beyond what my doctor considers reasonable.

I am pretty sure there is a 'standard' sunrise and sunset for Muslims around the world, so you don't end up with people above the arctic circle having to fast only three hours a day, while people in the Antipodes have to fast for eighteen hours. Rana can elucidate that, I am sure.

I hadn't known Ramadan was imminent, but yesterday was the first day in this lunation, the Moon having passed in front of the Sun during the previous 24 hours, and showing a very thin crescent last night, if you had the clear sky to see it. This event signals the start of the lunar month, of course.
