Tom, there are a lot of things that are not understood about gender identity issues or what causes it. The 'why' isn't necessarily something that there is scientific basis that is well understood at this point. As Dustin pointed out, children who are much younger come out to parents about this. As for doing something wrong, I would argue that you have done things RIGHT. If your child feels that they can safely come out to you about something like this, you've created a loving and supportive environment already. From here, the tough thing is figuring out where to go next, don't be afraid to get professional medical help.
