Rare as hens teeth, I should think.
Except I was once in an office where a man was showing his latest batch of photos round his colleagues. "Here's the little fellow dolled up as wee girl and this is big brother showing off his new skirt." As a visitor I was not in the circle, but from the glimpse I caught they were both very small, maybe 3 and 5. It was not Halloween, and nobody seemed surprised at all.
Also the 10-year-old son of a couple I knew played Cinderella in his school pantomime. Afterwards his mother told me "Rick in a dress was a sight to see!" Whether she meant he was so realistic or was a real travesty I cannot say. I never saw a photo or heard his father's comment either. But he was very much a hands-on dad, so he would have been supportive at least if not actually promoted the idea.
