Subject: What is it about Lesbians...?

I was reading a recently published WWII fantasy novel (Sisters For Victory, part one of a series about a British all female combat squadron...) and during the course of the story two characters are 'highlighted' by various mentions of them sharing a cigarette. Of course towards the end of the novel it's confirmed to the reader that they're in a Lesbian relationship.

Given that the link 'Female Smokers = Lesbians' blighted a number of smoking fetish stories back in the day it was a real surprise to see it popping up in what is most definitely not a smoking fetish story.

But it does raise the question, why is smoking so strongly associated with lesbianism?
