I think it is possible that there was a physical relationship prior to "all things" but as you said, Scully didn't committ herself to the relationship until then. The way she stops and looks at the sleeping Mulder mirrors the way he gently pushes her hair from her face. In "all things" we learned that Scully has had two sour relationships with co-workers. However, we also had a few lessons about the need to express your love, to free yourself from whatever is holding you back from total committment. Maybe that is what happened between them and the physical side of the relationship happened prior to "all things", say post "Millenium" or even "Milagro".
It is possible CC may never clear the time frame up for us. And that may be a good thing because it leaves us free to indulge our imaginations. As you told IslandGirl in another post, you love to read those "first-time fanfics". I think that in the Season 8 premiere we will get a couple of intimacy flashbacks. Some that we have already seen and one or two new ones. I am betting that there will be a scene in bed but no reference point on it and probably more of a post-event snuggle kind of thing rather than an overtly sexual one. I can see Scully at the end of the ep. going into Mulder's apartment being bombarded with images, finally curling up on his bed and then the scene and then a shot pulling away toward the ceiling of her all alone. Fade to black...titles...sound of all of us trying to get on-line at the same time.
