The producers came up with a great reason back when the first Doctor (William Hartnell) wanted to leave the role. The show was a hit so they wanted it to continue. The Doctor (he's never called Doctor Who) is an alien from a race called the Time Lords. They have mastered the ability to time travel and have also extended their life spans to thousands of years. When one gets very old or injured, you can regenerate. A new younger body is formed from your cells and viola! A new Doctor.

Each actor who has played the role brings his take on it but retains the basic idea of a time traveling hero who's eccentric and a bit unpredictable. Most actors have lasted on average 3 years in the role. One lasted only a TV movie. Tom Baker was the longest at 7 seasons-he's also the most famous-the one with the curly hair and long scarf.

The Doctor picks up companions along the way, people who meet him on an adventure and he invites them to travel with him, usually a young pretty woman. They come & go and their main function in the old days was to be the audience surrogate. The one the Doctor explains things to. The newer companions are more rounded characters and often are at odds with the Doctor over where & when he chooses to intervene.

Doctor #11 is on his first year now, I haven't seen it yet but will soon.
