The idea I've always had is simply that they were two different races. The smooth-headed Klingonese held power during the time of TOS. Sometime between TOS and the movies, the underclass Klingons, the bumpy-headed ones, rose up, massacred most of the Klingonese and took over. Some of the Klingonese, such as Kor, saw which way the wind was blowing and allowed themselves to be surgically altered. They renounced their own race, pledging loyalty to the greater Klingon populace.

I also have this theory that some of the Klingonese were able to escape to a remote world, where they set up a new society. They became the Breen.
