The reissue survey cards are essentially the same as the Binaltech ones, except for the following differences. Otherwise, fill it in exactly as you would a BT survey card.

#1 - You only need to fill in the product name, no BT number required.

#4 Please tell us your reason for purchasing this product.
(1) Because I collect Transformers
(2) Because I used to play with this toy once upon a time
(3) Because I like transforming robots
(4) Because I like toy vehicles
(5) Because robots are cool
(6) Because they come with file cards
(7) Because they're cool to display
(8) Other (specify)

#6 Option 1 is "Transformers Binaltech" instead of "Transformers Collection." Natch.

#8 This question asks you to specify up to three Transformers you'd like to see reissued.

...otherwise, the rest of the survey is the same as the BT one.
