Are you running it on a non-Intel computer EG AMDK6,K7 or similar? or it may be caused by your parallel port (some are troublesome)
Same thing happened to me and I put it down to the use of Pentium-specific code in the executable. I finally built the 'Fluffy' SX in-system programmer with a serial interface and had no problems since - added bonus is it also programs 16F84's in-system.
