Well, I haven't played the game, but if it can be scripted (if it's not in already) make -10 hit points the death threshold and 0 just unconcious. That way if someone falls in battle, whatever wasted them will move on to a healthy party member so there is a chance to revive the downed person. Then again, there should be some smarter creatures (or assassins) that will go to kill someone that is down. Still, ressurection should be very rare and cost quite a bit, either in terms of the wealth of the party or of the character himself.

Rare. Wizards should be highly respected from their years of dedicated study to be able to use magic. Sorcerors should be downright feared since their magic is innate. Priests should be held in awe for the miracles (magic) they can perform and bard magic should really be kept in check. Maybe have an island wide bards' guild that makes all members limit their public magic displays to things that can be explained as parlor tricks.
As for magic weapons, things such as +1 weapons should be very rare, but give us other useful items that are just plain rare :P
I'm talking about say a needle of mending, a spoon of excavation, a stone that produces something similar to torch light. Things that won't make a difference in combat but are useful anyway.

Full PvP all the time. I think our players are mature enough not to abuse this and are mature enough to accept the consequences of starting a fight. Just have guards react to it badly in down and try to break a fight up. Maybe have them do subdual damage, if that's possible.

No clue. Resting shouldn't take up a lot of time in a game, but it shouldn't be dismissed completely. I'll leave this one to the DMs.
