lost fuel efficiency -- Southeast Z71

Subject: lost fuel efficiency

i have a 95 ext. with 218000 miles that is in excellent shape. In the last couple months mileage has gone to shiott, about 200 to a tank. I have done all the obvious, tune up, fuel filter, nothing has helped. And there is no check engine light. Horsepower is the same and good, but i can smell that she's running rich. I'll try to describe whats going on- from a start it feels she's being held back and then late & hard to shift, once to third and overdrive it runs fine. Any help is greatly appreciated
