This sounds more like a psychological problem than a medical one. It centers on snags around potty training. It will, more likely than not, begin to filter through his personality into other areas.
A snag around potty training happens through the interplay between a child's sensitivities and a mother's impatience and overall tone towards a child's bowel movements. Both are natural occurrences, so a blaming attitude towards either party would be unproductive. Depending on predispositions, the child will develop strategies (from a limited array of available choices, fecal holding being one) to deal with his perception of his mother's attitude toward his bowel movements.
This can get complicated without clarity. If you can afford to use a child psychologist, with the right psychologist it would be resources well spent in terms of the child's future well-being.
