My son who is almost 7 had the exact same problem. I Know its late and your child has probably grown out of this but in case some one else needs help here goes: I didn't go to a doctor. I gave him a pedia enema to get the biggest one out first and in two days I said go sit (took one hour of sitting) and poo or you get another enema. An itty bitty thing popped out. I made such a big deal that he was beaming from head to toe and I said you did such a good job every time you go poo I will give you a reward. Every other day I said sit on the toilet until you go. It took less than a month and now he is doing well and my younger son who is 3 is totally trained too!
I figured the cost of diapers, and underwear is still cheaper to buy even if you throw one out, so let him keep his dignity and let him wear underwear not diapers. If you are not comfortable with the enema part let a dr do it or wait until he does go on his own then do the big deal thing. I gave mine candy, Ice cream, Dad's lunchbox items, you can even ask what he wants After he goes of course. I have slowly weaned him off of giving rewards now. Hope this helps.
