I had this exact problem with my six year old son last summer, right after he turned five. He had been pottytrained for two years and had a few episodes of constipation from not wanting to go. What helped wasn't that fun, but we haven't had issues with it since. During a t-ball game he began crying in pain from his stomach hurting. I took him to the ER and told the doctor about him holding in his bowels for a week at a time. The doctor took a sample of his bowels (by inserting his finger in my son's anus) to rule our any other problems and when he checked out fine, I was instructed to give him an enema (liquid stuff up his bottom). He cried and fought me but I did it anyway and told him that I had to do it because he held his poop in for too long and if he didn't want to poop on his own, the doctor said I have to get it out with enemas. Ever since then, he's been fine. That was a year ago. He was old enough to understand that he wasn't in trouble or anything, it was a choice that he made to hold it in or get it out and he'd rather do it on his own than have me use an enema. Kind of extreme, but I felt it was the only option at that point. We also give him fiber pills everyday. He has always eaten healthy, veggies everyday and all that, but for some reason, he didn't want to go. Since he doesn't need any help using the potty, I used to ask him when he had last gone if I didn't notice him going to the potty everyday. I would remind him about having to go to the doctor and the doctor sticking his finger in him and me giving him an enema. Kind of a scare tactic, but it works and I use it very calmly and matter of fact. I hope that helps.
