Geez, sounds like you've about tried everything. Have you considered shaving the puff down, not that it will stop the fleas, but it will be easier to find them.

You might also start adding Brewers Yeast supplements, as well as some all natural flea deterrent products (in addition to the Frontline Plus or other similar products) that contain Neem Oil. I use an all natural neem oil product for my epileptic dog and he has no problems with it.

You might also try spread diatomaceous earth around your hard & even in your carpet, dog beds, etc.
