I've had experience with this before. My gelding came in one day and I noticed his sheath was enlarged. I called my vet and she came out, sedated him, and pulled out and rinsed everything. I took maybe 2 days before the major swelling went down and then another week until it was back to normal. One thing my vet recommended was that everytime I bathe him, clean it all out, not just the surface and even if your not bathing very often, clean it out.
