It was Al himself as a POW during his Vietnam days. That explains why Sam says "You could have been free." Al makes the supreme sacrifice of letting Sam save his brother instead of letting Sam rescue him (Al) and thereby saving his marriage and changing his whole life by bringing him home five years earlier. I haven't seen the episode in quite awhile, so I can't give you more details, but that's the idea.
