I'm pretty sure that wasn't the last transmission of the Osiris, it wouldn't make sense if it was.
Niobe/Ghost picks it up at the beginning of ETM, its then sent to Zion. Note: Neo isn't in Zion at that time.
Next comes the meeting of the captains, which is where Reloaded starts, Neo, Trin and Morpheus get there late. Then it is explained to the captains about the machines digging.
Ballard says he'll stay and wait for the Oracle, then the Agents arrive. Ghost/Niobe help everyone out.
Then there's a clip of Ballard fighting Seraph, presumably he then sees the Oracle.
It is soon after this that Ballard visits Neo in Zion. The disc probably contains the loaction of Seraph's tea house, so Neo can see the Oracle.

And no it's most definately not the same one he gave Choi, we don't know whats on that.
