In answer to your 2nd questions, most orthos are willing to work with you on the payment plan. I would ask them to work with you and set up a plan that is acceptable to them and works for you.

My plan was set up based on my Flex Healthcare Plan at work, where I can use money in my flex account for health costs tax-free. So I set up a simple plan to pay 1/2 up front (all covered and paid thru my flex plan, which takes regular deductions from my pay check)and the other 1/2 at the same time next year.
