Sometimes an expander can be used to bring the lower jaw forward, but not all orthos offer this and it won't be successful in all cases. Many orthos suggest surgery - it was suggested to me by two orthos but I decided against it because I have no jaw pain/locking/TMJ or anything like that, so I didn't want to mess with a perfectly good jaw.

There is debate about whether a "perfect" bite is necessary. An 'incorrect' bite can cause uneven wearing, jaw problems, migraines etc but not always. Some people have a misaligned bite for their whole life and have absolutely no problems. Other people have lots of problems. For myself, I have an overbite (which I still have, as I only had braces on the top to straighten a few teeth) but have never had any problems. Although one ortho I saw insisted that I needed a perfect bite, when I said that surgery wasn't an option he said, "Chances are you'll never have any problems if we don't fix your bite."

So (long answer to a short question)... you may find that orthos will tell you that you need surgery. My advice is, get several opinions and always ask "What are the implications if I don't?" and "Are there any other options?" Sometimes surgery is absolutely necessary but sometimes it's not.
