I have to answer YES to all the above... I would question the value of the dog, if this happened. It is possible, IF the dog has not been shown these things in training.

Has anyone ever witnessed these things?

I have witnessed a dog coming off of a bite in a trial, and going back to the handler... it's not pretty, but the dog was not ready for the level of pressure that it was shown. And, that sometimes happens in a trial.
