The reason the city is able to offer a buy out to employees opting out of health insurance coverage is because of the saving to the city. The average plan cost around $12,000 a year. Giving the employee $250 a month comes out to $3,000 for the year. That is a savings of $9,000 on the yearly policy. Hope this clears up some of your confusion.
