This happened to me a few years ago. When I checked into why this happened, HR at my company informed me that they were not required by law to take out the percentage of taxes from where you reside. They are only required to take out the percentage from where the company is located. For example, I used to work in WB Twp. The taxes there are 1.5% and that is all that they were required to withhold. I live in the city where the taxes are 3%. Therefore every year I ended up paying the city. It sucked.
You may want to talk to your HR and ask them if they can withhold a greater percentage from your paycheck. This eventually worked for me.
Now I work in the city so I don't have to worry about this any longer. Hope this helped.
