I think it's an interesting concept that could be very successful. I also agree with Cass and liked the layout of the website.

One of the concerns I would have is the potential for a newer patient to misdiagnose himself. For instance, after reading the descriptions, the patient could be experiencing wheezing which could lead him to believe he either has allergies or asthma. There are 3 options for him to choose: the allergy eval, asthma eval, or wheezing/asthma symptoms.

Let's say he chooses the asthma test because it's the least expensive, and he's got a friend who wheezes, and that friend has asthma. Because the friend wheezes and has asthma, he thinks it must be the same thing. So he selects the asthma eval, gets the pulmonary function test (PFT) done, and the PFT comes back normal. Some allergy testing is included in that package, but for argument's sake we'll say it's not enough. Now the doctor will probably recommend for him to go for more allergy testing. Because he didn't choose correctly he's going to have to fork out another ~$200 on the allergy eval.

If he had initially seen the doctor, and described his symptoms to the doctor, the doctor would have (hopefully) led him in the direction of the correct test the first time saving him a few hundred dollars. If this scenario were to happen, it has the potential for unhappy customers if they're left to make the decision on what treatment they need, and if they choose incorrectly.

I'm not sure how likely that is to happen, but I do think it would be possible.
