I've just finished my third tie. I just used cotton and it has worked great. Didn't use EMLA cream either, but trust me, if you use alcohol wipes on the frenulum/head area the stinging pain from that will dull any needle pain. I'd didn't burn the needle, as I just wiped it in alcohol and then Betadine. Also swabbed the frenulum area with Betadine. Just keep using Betadine a couple of times each day till it breaks the fren and then heals. Also keep it clean after urinating.

My only other two tips:
1. It takes a surprising amount of force to poke the needle through
2. Don't break the thread when tying. On my third go, I broke the thread when doing the initial surgeon's knot, so I couldn't put a safety knot in. After 12hours I could tell it had loosened off, so I had to put another tie in. Cause t was so close in time and distance to the first knot, the area was extra sensitive and discomforting. Also when the tie broke the fren, the initial loose knot had to be snipped away. No pain and didn't change the result, but a little bit more worrying to the nerves than when it all just works perfectly.
