I just gave two shots this morning, both to young attractive girls. The first girl was 25 and got a shot of rocephin. I told her it had to go in her hip. NO problem and she stood up pulled down her jeans to her ankles and bent over. No panties My experience is that men usually barely expose their butt cheeks, young gilrs have no problem showing their butts and fat women tend to barely expose their butts.

Whether it was a shot or a rectal temperature regardless of age, most of the time, I was on my stomach, and my pants and underpants had been lowered to just behind my knees. I once got a RT while bent over the front of an exam table, followed by an injection in my left cheek. Pants/underpants were around my ankles, simply because I was partially standing and they, upon being loosened/lowered continues to drop on their own. That was the only time when I wasn't face down for either the temperature taking or shot(s)

Oh, the only other time where it was different for shots, was when as a kid I got the Pasteur treatment! I was on my back, my pants lowered to my upper thighs, and my shirt lifted up, and depending where the previous day's injection had been given, I might have had my underpants slightly lowered so my lower abdomen could be accessed used as an injection site. Rabies shots, circa 1960s were not pleasant!
