Subject: NG, sand traps and ball position on all shots

I am learning NG. How does one hit out of a sand trap using the NG set-up and swing?? Also, what is the ball position for each club in general with respect to the feet/body?? One reference noted the ball should be outside the left shoulder for the driver but did said nothing about other clubs. And finally what is the relationship of the club head to the ball during set up?? One reference had it 12" in back of the ball for a driver, and 4" behind for the wedge.
