I assume he isn't autistic. You've probably been over all the usual possible causes, but since you don't seem sure it's dyslexia, you haven't taken him in for an fMRI yet. That will show whats going on in his head when he's trying to read.

Something else to consider - what if he's a savant of some sort? Kids naturally have very limited patience, so anything thats 'stupid' can frustrate them and they can find it difficult or impossible to pay any attention.

I can send you a few Nooalf charts to try. If he is being turned off by contradictions and bad lojik, this could work well for him. just email a request to nooalf@aol.com
