I have a 7 year old son with many of the same problems you've described. He's very bright and I never understood why he wasn't doing better in school. He created distractions in school to avoid doing the work like dropping his pencil box over and over, losing his things. He has a lot of trouble with directionality, handwriting, and word recall. His teachers wanted him tested for ADHD too. However, when I talked to my pediatrician, he suggested getting a full psychoeducational evaluation. Thank god we did, because he has dyslexia. His attitude toward school has changed dramatically this year. The psychologist wrote up suggestions for accommodations in the classroom like letting him do math orally and he's getting an hour a day of SLANT reading which is an Ortin-Gillingham method that you use for Dyslexic students. The school was not willing to do anything until I had the test in my hands. Most kids fall through the cracks until 4th grade when they are two years behind in reading and the school finally steps up. Good luck!
