Like someone else said already, in a videogame "death" will happen more often than it would in a pen and paper game. Even fighting stupid skeletons there can always be the occurance of 2 skeletons charging the weakling caster of the group and beathing him down before anything can be done about it. There is a Ressurection spell and it is allowed only to high level characters. Since this is going to be a low XP campaign and all the characters will start at level 1, it will be very unlikely that any party will have easy game at resurrecting any comrade easily. They will have to look for help... maybe the local Cleric wont' be that close and they will have to travel to the Church while somebody guards the fallen body... the Church may require some donation.. I see a lot of possibilities here. Short version: let us allow the Resurrection spell, and use the DM skills and powers to make death a feared thing in any case.

I believe that the spell effects should be IC, of course.

Full PvP... we are all mature players. And how can I PK for m4d l3w7 without pvp anyway?

Hmm since I don't have the game yet (will be here tomorrow) I can't comment on this one. I'm guessing that the normal game mechanics should apply.

Kathy (or was it Gina) put up a good point about server stored characters... let's say that at the planned day the "server" is not available, nobody can play the campaign. If we have the characters on every player's machine, we can just switch to another IP and play nicely. I would not think that any of the people in this group would cheat anyway. Again, I don't have the game yet so I am not sure on what the real differences are between server stored and player stored characters :)
