No clue! I think some of your questions can be answered after a test run though.

1) that totally depends how often your char dies. If you really get killed too easily then I opt for the unconcious thing, but I think we actually have to find out in a test campaign

2) I don't understand the question about those IC and OOC effects. What my char sees ingame that exists.

3) we should be able to attack other party members.

4) since resting is so quick I prefer to see it as "taking a break" rather than lying down and sleeping for 8 hours. I think if the party wants to sleep and rest in an RPing sense then they have to build a camp, set watches, etc.
