Funny you should mention this... just this morning I was listening to AT40 on the net, Energy FM out of Canada, and they ended hour 2, went into their station promo, and proceeded to start hour 1 again. Fortunately, the board op caught it and after about 30 seconds started hour 3.

WFLZ here in Tampa used to screw up the show on a regular basis, cutting out segments in the first or second hour, playing segments from the previous week or month (one time during the 1998 yearend show, they actually started off the show with a regular weekly countdown segment from about 2 months prior; what the hell was that even still doing in the studio?). Fortunately they're nowhere near as bad as that anymore.
