I also have a DSR100, although mine is still working.

I've seen your symptom 1 for a long time. I can't remember if it's the result of a long-ago firmware upgrade or not.
In my case I find it's best to leave the tape in when the system is powered up and not try to eject it. If the power loss happened when the unit did not have a tape in it, then I have to cram a tape into it during the few second window when it's on. When I don't have a tape in my unit, then I also see your symptom 2 before it shuts itself off.

I've never seen your symptoms 3 and 4 although I also have a two dish antenna. I wonder if they could be related. Maybe there's a problem with the digital to analog conversion circuit which both paths (tape to TV, dish to TV) have to go through? Other than that I don't have any ideas.

I don't know of any upgrade by Dish for over a year or more now. My guess is that they've pretty much stopped supporting this unit.
