Sandi,
I have a couple of clients in an assisted living facility, which means they're home when I come over for the dogs. We have a very clear understanding that I am there strictly for the dogs, and that if for any reason they manage to keep me there longer than 30 minutes, it shows up on their next bill. I will, however, take a bag of garbage out with me as I leave, or pick up something they dropped on the floor and can't reach, etc. For one client, I always pour her a glass of milk just before I leave. I'm also willing to run errands for a specified hourly rate, but they usually have someone else do that for them. I just tell them up front that if they should need me to run to the store or something, I need advance notice so that I can fit it into my schedule, and I'm paid well for it. I've actually come to really enjoy these clients! ;-) Good luck!
