I'll answer each in turn...

1) If you find yourself drowning, first make sure you're set on run so that you move as quickly as possible. Then just try and swim out. If you're not a good enough swimmer and you've got some extra weight, drop it and try to swim out again. If you're still stuck, watch the sides of the river carefully on exit. Some spots don't require you to swim up before you can move to the shore. These are few and far between though. Last ditch effort, get nekkid and try to swim out again. Still doesn't work? You're fish food.

I guess it ought to be said that there are places in the world where a clever/lucky player will be able to get out even if their character can't swim.

2) The best way to avoid falling in a river like an idiot OR falling off a cliff is to use peer as you move about. Specifically, a command like peer down like Malik said. If you can see something "down" on peer then use the exit command to see what kind of terrain is in that direction. For example, if it says there is a river to the east on exit and you can see an area to the east and down on peer, then you will, more likely than not, sink down if you move east. If exit says there is a river to the east but peer east down doesn't show anything to the east and down, then it must be a shallow river and you'll be okay to move there. If it's a cliff to the east and you see a room on peer east down, then you'll probably have a fall. I think you get the idea. Oh yeah, if it's night and you don't see anything on exit but you do see are room down near you then just assume the worst. Stick to that and you'll be safe.

3) Did you notice the majority of the equipment was clumped up in about the same spot? I bet you could work out how many hit points a centurion has by the speed of the river and the rate at which you lose hit hitpoints while in a river.

4) Yes. Works the same as climb as far as I can tell.
