How do you grandfather a contract while maintaining a salary cap for everyone else?

Say each team has an annual cap of $100 million. You have 40 guys to pay within that cap. Your team has an abomination unto God like A-Rod whose contract is grandfathered, so how much does his salary count against the cap? Does your team now have $100 million to pay 39 other guys, and A-Rod doesn't count? Or do you figure out an "average" salary and attribute that to A-Rod, which still gives his team a big financial advantage until his contract expires? "Hey guys, we're paying A-Rod $20 million this year but he'll only count $4 million against the cap so we still have plenty to spend!"

I just don't see it.
