Subject: my horse gets hot spots on his neck

My grey 12 year part arabian occasionally gets hot spots on his neck. The will appear as a roundish area, 3 - 6 inches across and are so hot they are wet with sweat - even in the winter. They are so itchy he will rub them raw. Occasionally this is accompanied by a puffiness in the legs. He was being fed a mix of grass, alfafa hay and I have switched him to alfafa/grass hay cubes. I don't normally feed him any grain unless he is working. I would appreciate any information on this matter.
