Well...

1. The kid was freed, and since he remembered, he gave Neo a spoon as a reminder of what he can do in the Matrix

2. Agent Smith can assimilate people inside the Matrix, and if he transfers his mind, if you will, to a free mind without changing that person's appearance, he can switch from Matrix to Real World without problems.

3. Yes, it's called government.

4. I thought about that for a while, and I did actually think that it was another Matrix, but all other evidence points otherwise. If the Sentinels are controlled from outside the Matrix, why can't Neo screw around the Matrix from inside the Real World? He just has to decide the Sentinel control has to stop, and poof.

At least, I think. ^^'
