I was faced with the same decision between extracting four premolars and jaw surgery. I chose to go with the extractions because I was worried about the potential complications associated with jaw surgery. I had my premolars extracted several months ago, and the gaps are closing up nicely. The extractions were much less painful than I thought they would be.
