in the very beginning of the book, before the twins are even conceived, Alia is very powerful already. She is the high priestess of the religion of Muad'Dib and she is fifteen. She sits at council and she is the prominent religious figure.
Presumably she is an unknown quantity to Paul. I suppose he used her for her talents, while keeping her close in order to keep her under his control and under his observation. I guess when you are ruler of the known universe you might want to keep tabs on your prescient, renegade Bene Gesserit Pre-born Fremen-raised sister.
