If he suffers from depression, his smoke is only going to make it worse. If Prozac didn't work, there are a whole ton of newer drugs, all slightly different, that may. A good doctor will help him find the right combination of meds and therapy to combat this.

But I think you're right - he needs to commit to you before you can commit to him.
