The fireball Willow sends in Grave to kill Johnathan and Andrew (and Dawn and Xander) wasn't (primarily) intended to kill them but to get Buffy out of her way so if there's that element of uncertainty/questionable intent would that cause the chip to fire? Sort of like closing your eyes and firing a gun at random - is the intent to harm expressly present? I can't recall any other, more direct firepower being used but the whole thing is a matter of opinion obviously.
