There's a place in the episode where John says " I never should have let him go up on the mountain by himself to plant those seedlings." I think it implies to grandpa having a heart attack while up on the mountain. Because keep in mind, they were always making reference to Grandpa's bum ticker as he used to call it, after the episode where he had the heart attack. John V.
